Medical Corporation Tokushukai, Yao Tokushukai General Hospital shall handle privacy information of patients, visitors, hospital staff members properly and strictly as one of fulfillments of the hospital principles, “Hospital where people is relying on its life in comfort” and “Hospital protecting people’s health and life”.

The hospital has set the Privacy Policy as a specific assault of the principle and is implementing privacy policy management system in compliance with JIS15001. Based on these, we properly utilize and manage privacy information and assure staff member to understand and recognize the significance of these privacy information.

• We shall make staff member to understand the significance of privacy information, thus it shall build out a private information protection management system for proper use and protection of the private information, and steadily implement it. Furthermore, we will maintain and continually improve this system.

• 2．Collection/ Utilization / Provision of the privacy information

• We have recognized the significance of privacy information obtained from patients and user and from the standpoint of medical and nursing care service provider. We establish the administrative structure for protecting privacy information corresponding to each operation, and handle private information for its collection, utilization and provision properly to meet certain regulation and law.

• To secure accuracy and safety of the private information, we implement safety measures and make effort not to lose, damage, destroy, leak the private information, also protecting them from unauthorized access.

• We respect patients’ and user’s rights as the information subject of privacy information. When disclosure, correction or deletion of the privacy information, or rejection of utilization or provision of the self-information are required by the information subjects, we will respond to the inquiry as is reasonably possible according to conventional wisdom and customary practice.
