Admission Guide

At Admission

We understand and feel all family member worries very much about family’s admission.

We sincerely wish that the patient will get well and discharged from hospital without further delay.

There is a thing which patient and family member need to understand at the admission. This hospital is designated as Emergency Medical Care Hospital, so that there are patients need to have hospitalized treatment in acute phase. We make the utmost effort to organize and prepare for the admission whenever necessary.

For this reason, our sincere hope is that patient symptom is getting well without further delay and discharged from hospital. However, depend upon the role of the hospital and patients’ course of the symptoms fundamentally, we may ask that the patient is transferred to home care, or changed the hospital which is providing chronic medical care.

  1. For Social Insurance Insured Person (Principal), Social Insurance Dependents (Family member), and National Health Insurance Insured Person: Obligation Fee is set in each system.
  2. Eligible person under the Law of Health and Medical Service for the Aged: Obligation Fee is set by law.
  3. For medical expense other than insured treatment (Private Room, Documentation Fee, etc.), please note that the consumer tax is charged.
  4. For private room usage, following fee is added per day.
    Private Room 8,800 Yen / Day
    9,900 Yen / Day

  5. Charge calculation is initiated at midnight.

    As the hospital is assigned as Emergency Designated Hospital, we may request to secure private room for emergency admission and medical treatment for seriously injured patient, therefore depend on the symptom, there may be the case where patients need to change room after admission.

Admission Process (Guide for the current day)

On the admission day, please prepare filled-in and sealed necessary documents, registration card and other document necessary. With these documents, please complete the sign-in before designated time.

Preparation for admission

Please bring the following item at the time of admission.

  1. Documents
    Registration Card, Personal Seal, Health Insurance Card (person having Insurance Card for Aged People or other Medical Card, bring them together) shall be presented at the reception
  2. Registration Card and medicine – Please bring all currently taking or in-use medicine.
    (Oral medicine, Eye-drop, inunction, Suppository, Fomentation etc.)
  3. Necessary stuff for daily life
    • Toiletry Kit (*1)

    • Towel (*2)

    • Cup

    • Water Flask (*3)

    • Slippers, Sports Shoes (*4)

    • Night Clothes

    • Underwear (*5)

    • Cardigan, or similar clothes (*6)

  • (*1) Toiletry Kit: Wash Bowl (use when bathing, face wash, or wiping body), Toothpaste, comb, Soap, Shampoo, Conditioner
  • (*2) Bath Towel: 2 pcs, Towel: Approx. 5 pcs are recommended.
  • (*3) Please avoid electric kettle
  • (*4) Please use slippers while in hospital. Recommend using quiet and non-slippery one.
  • (*5) Please prepare 3 to 5 sets.
  • (*6) Please prepare overwrapping clothes when it’s cold.

※Night Clothes can be rent at 110 yen per day. During summertime, it is exchanged 3 times a week. During wintertime, it is exchanged 2 times a week. Please submit “Hospital wear IOU written acceptance form”.

※Minimize personal belongings and put your name on them.

※During admission, you are in communal life. Try to get organized, do not make others annoyed.

Things we’d like you to keep in your mind during Admission

During admission, following points and instructions from your primary doctor or nurse should be kept and followed for not only yourself but other patients are able to receive enough treatment with good cheer.

  1. Usage of electrical instrument other than hospital equipped one should be consulted with Nurses.
  2. When using TV/Radio/CD Player, use earphone not to disturb other patients.
  3. Drinking and bring-in alcoholic drink is prohibited.
  4. Entire hospital premises are NON-SMOKING set by Health Promotion Act (Smoking in Hospital premises are prohibited). For those currently smoking, it is recommended to stop smoking during admission.
  5. Do not carry any valuable goods, whenever possible. Especially, put your utmost caution for cash money, and make best effort not being stollen. We at Hospital waives all responsibility and compensation for stolen goods.
  6. Patient outing or sleepover shall be accepted by patient primary doctor. (Please consult with Nurse).
  7. When patient breaks a glass or other equipped item in hospital ward, patient should compensate at its cost.
  8. Please do not call patient except for urgency case. Specially, later than 21:00 hour, do not call patient in any case. Regarding the usage of mobile phone, silent mode is recommended, for not to make other patient annoyed.
  9. We sincerely decline to receive any gift or thoughtfulness to hospital staff member.

Please understand the following point in advance.
When patient do not follow above things or instruction which we would like you to keep in your mind during admission and is disturbing hospital operations, patient may be discharged from the hospital.

Payment of Admission Charge

  • Admission charge is closed at each month end and invoiced after 10th of the next month.
  • Other than private room patient, the charge is invoiced once a month.
  • Patient in private room, charge is invoiced twice a month.

    1st time: closing at 15th of the month and invoiced after 20th of the month..
    2nd time: closing at end of the month and invoiced after 10th of the next month.

  • Invoice is delivered to your hospital room. When received, payment shall be made at 1st floor Admission Counter within 5 days.

    Monday thru Saturday: from 09:00 to 17:00
    Sunday and National Holiday: from 09:00 to 17:00

    Handling other than the above hours is made at 1st floor Front Counter.

※Invoice for discharging patient shall be made when patient is discharged.

※When charge is unable to calculate at discharge, hospital will contact on later date.

There is no admission deposit.

Informing the Medical Condition

Doctor only informs medical condition of the Patient directly to him/herself or to family of the patient. Even if urgent request comes up, there may be a case where it is unable to fulfill request due to surgery or in-middle of treatment reason. So that in case medical condition information is requested, please ask for the primary nurse in prior.

※We are unable to answer the inquiry regarding disease name or medical condition over the telephone.

※We sincerely withdraw the explanation of disease name or medical condition of the patient to any patient relatives, company, acquaintance, friend.

Please make the explanation directly from patient itself or family member.

Inquiry for Medical Certificate and Documents

In case Medical Certificate or other documents is required, please let us know ahead of time. (The defined document fee is charged.)

Nursing Care

We extend best possible cordial treatment to every patient depend on his/her condition. Please do not hesitate to ask us anything.

Meals

Nutritionist is preparing the energy calculated menu for every meals.

  • Supper:18:15

※We are preparing the suitable therapeutic food for kidney disease, diabetes, liver ailment and high-blood pressure patients. So that we sincerely decline self-catering or bring-in foods.
